Accident Summary:

ON 15 APRIL 1969 A UH-1B HELICOPTER SERIAL NUMBER 64-14043 CRASHED INTO A BRICK BUILDING SOUTHWEST OF THE END OF RUNWAY 24 AT BAN ME THUOT CITY AIRFIELD. THE HELICOPTER HAD BEEN ON A ROAD RECONNAISSANCE FROM NHA TRANG TO BAN ME THUOT. IT ARRIVED AT BAN ME THUOT CITY AIRFIELD AT APPROXIMATELY 1500 HOURS AND REFUELED TO 900 POUNDS. THE HELICOPTER WAS HOVERED APPROXIMATELY 50 METERS WEST OF THE POL POINT AND PARKED, WITH ONLY THE AIRCRAFT COMMANDER AND PILOT ABOARD. AT 1715 HOURS THE HELICOPTER WAS RELEASED TO RETURN TO THE ^48TH AHC^ AT NINH HOA. A HOVER CHECK WAS COMPLETED AT TWO FEET ALTITUDE WITH 6400 RPM. THE TOWER CLEARED THE HELICOPTER FOR TAKE-OFF ON RUNWAY 24 WITH WINDS AT 320/6 AND OAT AT 34 DEGREES CENTIGRADE. THE AIRCRAFT COMMANDER HOVERED THE HELICOPTER TO THE RUNWAY AND COMPLETED ANOTHER HOVER CHECK AT ONE FOOT ABOVE THE GROUND WITH 6400 RPM. A RUNNING TAKE-OFF WAS INITIATED AND THE SKIDS REMAINED ON THE GROUND FOR APPROXIMATELY 50 FEET. RPM DROPPED TO 6000 AND THE AIRCRAFT COMMANDER LOWERED THE COLLECTIVE PITCH. THE HELCOPTER WENT THROUGH TRANSLATION AND BECAME AIRBORNE. THE AIRCRAFT COMMANDER FLEW THE HELICOPTER THROUGH AN OPENING IN TREES TO THE LEFT OF THE RUNWAY. AIRSPEED WAS APPROXIMATELY SIXTY KNOTS AND RPM WAS 6300. HE LOWERED THE NOSE TO GAIN AIRSPEED AND IMMEDIATELY EXPERIENCED A LATERAL VIBRATION THROUGH THE CONTROLS AND A LOSS OF RPM TO 5700. AS THE HELICOPTER FLEW THROUGH THE OPENING IN THE TREES, THE SKIDS STRUCK THE ROOF OF ONE BUILDING AND THE MAIN ROTOR BLADES STRICK THE ROOF OF ANOTHER HOUSE ON THE LEFT OF THE FLIGHT PATH. THE HELICOPTER TRAVELLED ANOTHER 50 METERS, THE MAIN ROTOR IMPACTED A TREE ON THE RIGHT OF THE FLIGHT, AND THE HELICOPTER CRASHED INTO THE BUILDING. AT IMPACT, THE GUNNER WENT THROUGH THE LEFT WINDSCREEN. A SMALL FIRE WAS SPOTTED IN THE ENGINE COMPARTMENT BUT WAS EXTINGUISHED BY THE GUNNER. THE PILOT DEPARTED THE HELICOPTER UNDER HIS OWN POWER AND ASSISTED THE CREW CHIEF AND GUNNER TO REMOVE THE AIRCRAFT COMMANDER FROM THE HELICOPTER. A HELICOPTER FROM THE ^155TH AHC^ LANDED AND MEDIVACED THE CREW TO THE HOSPITAL AT BAN ME THUOT.
