Accident Summary:
AIRCRAFT ^68-15524^, A UH-1H, WAS ASSIGNED THE MISSION OF TROOP EXTRACTION WHICH BEGAN AT 0735 HOURS, 8 DECEMBER 1971. AFTER WEATHER DELAYS WHICH RESULTED IN ONE HOUR OF FLIGHT TIME THE MISSION WAS RESUMED AT 0930 HOURS. TWO SUCCESSFUL EXTRACTIONS OF TROOPS WERE MADE AT TWO DIFFERENT PICK-UP ZONES. THE ACCIDENT OCCURRED AT THE TERMINATION OF THE APPROACH TO THE THIRD PICK-UP ZONE. THE AIRCRAFT WAS THIRD IN A FLIGHT OF FIVE MAINTAINING THIRTY SECONDS SEPARATION. THE APPROACH COURSE INTO THE PICK-UP ZONE WAS FROM NORTHWEST TO SOUTHEAST WITH THE WINDS REPORTED AS LIGHT FROM THE EAST. VISIBILITY AT THE TIME WAS A MINIMUM OF THREE TO FIVE MILES. THE AIRCRAFT COMMANDER, '1LT. ROBERT B. ALLEN^, WAS AT THE CONTROLS AND BEGAN HIS APPROACH AS CHALK 2 LANDED. HE ALLOWED THE APPROACH TO BECOME STEEP AND SLIGHTLY FAST. THE PILOT, ^WO1 RICHARD L. NORDSTROM,^ INFORMED THE AIRCRAFT COMMANDER THAT ALL INSTRUMENTS WERE WITHIN NORMAL OPERATING RANGES AT THE START OF THE APPROACH AND AGAIN ON SHORT FINAL. THE CREW CHIEF, ^SP-5, CHRISTOPHER R. SEITZ,^ AND GUNNER, ^SP-4 ALBERT E. DAVIS,JR^, INFORMED ^1LT ALLEN^ THAT THE AIRCRAFT WAS CLEAR DOWN AND THAT THE TAIL ROTOR WAS CLEAR. BEFORE THE AIRCRAFT COMMANDER COULD PULL IN POWER AT THE TERMINATION OF THE APPROACH THE RIGHT SKID IMPACTED THE GROUND CAUSING A LOSS OF CONTROL. THE AIRCRAFT ROLLED TO THE RIGHT SKID RESULTING IN THE MAIN ROTOR STRIKING THE GROUND, AND THE AIRCRAFT COMING TO REST ON ITS RIGHT SIDE. THERE WAS NO FIRE AND NO INJURIES AND THE CREW EXITED THE AIRCRAFT WITH MINIMUM DIFFICULTY. THEY WERE EVACUATED FROM THE SCENE BY THE TRAIL AIRCRAFT.
