Accident Summary:
^UH-1H ARMY HELICOPTER 67-17154,^ WAS INVOLVED IN AN ACCIDENT DURING A DCS MISSION FROM ^NINH HOA, RVN TO NHA TRANG, RVN^. THERE WAS A CREW OF FOUR (4) ON BOARD THE FLIGHT. A WEATHER REPORT FROM ^NHA TRANG^ TOWER IN THE AREA OF THE CRASH AT ^0955 HOURS^ WAS 3500' OVERCAST, 2500' SCATTERED, VISIBILITY 11 MILES, AND WINDS 320 DEGREES AT 8 KNOTS. THE MISSION OF ^67-17154^ WAS TO PICK UP FIVE (5) ^KOREANS FROM DIVISION PAD AT OP 968815,^ AND TAKE THEM TO ^NHA TRANG, RVN^. THE AIRCRAFT WAS FLYING NORTHEAST TO SOUTHWEST AT 1500', 80-85 KTS. PIC WAS IN PROCESS OF SWITCHING TO ^NHA TRANG^ TOWER FREQUENCY WHEN A LOUD SNAP OR POP WAS HEARD AND AIRCRAFT EXPERIENCED SEVERE LATERAL VIBRATIONS. PIC TOOK CONTROL OF THE AIRCRAFT AND INITIATED A 180 DEGREE AUTOROTATION, TO MAKE AN AREA OF OPEN ROAD WHICH WAS APPROXIMATELY SIX (6) METERS WIDE WITH NO OBSTRUCTIONS. PIC ATTEMPTED TO CALL ^NHA TRANG^ TOWER, BUT WAS UNABLE TO DO SO. PIC INSTRUCTED PILOT TO SWITCH RADIO TO GUARD FREQUENCY. THERE WAS SOME CONFUSION ON THE PILOTS PART AS TO WHAT THE PIC WANTED HIM TO DO. UPON ROLLING OUT 180 DEGREES AUTOROTATION PIC NOTICED SEVERE LOSS OF ROTOR RPM, AND INITIATED A GENTLE FLARE IN AN ATTEMPT TO REGAIN ROTOR RPM. AT APPROXIMATELY 30^-50' ABOVE THE GROUND AIRCRAFT HAD ALMOST NO ROTOR RPM AND FELL STRAIGHT DOWN LANDING WITH THE NOSE OF THE AIRCRAFT 30 DEGREES T TOHE RIGHT OF THE ROAD. THE AIRCRAFT HAD LESS THAN TWO (2) FEET OF FORWARD GROUND RUN. THE MAST SNAPPED AND THE HEAD AND ROTOR BLADES WERE LAYING PARALLEL TO THE LEFT SIDE OF THE AIRCRAFT.
