Accident Summary:
THE AIRCRAFT WAS ON A FIRE FLY MISSION OVER CAN THO AAF. APPROXIMATELY 40 MINUTES INTO THE FLIGHT, THE AIRCRAFT COMMANDER NOTICED A LATERAL VIBRATION AND STIFFNESS IN THE ANTI-TORQUE PEDALS. HE CONTACTED THE MAINTENANCE OFFICER AND REQUESTED THE AIRCRAFT BE TEST FLOWN. THE AIRCRAFT WAS LANDED AND THE MAINTENANCE OFFICER TOOK OVER THE TEST FLIGHT WITHOUT SHUTTING DOWN THE AIRCRAFT. THE COPILOT/GUNNER ON THE FIRE FLY MISSION, WENT WITH THE MAINTENANCE OFFICER ON THE TEST FLIGHT. THE PILOT CALLED FOR CLEARANCE TO HOVER AND TAKE OFF. FOLLOWING THE HOVER CHECK THEY TOOK OFF TO THE WEST, AND EVERYTHING WAS NORMAL DURING ONE TRIP AROUND THE PATTERN. ON INITIATING A LOW PASS AND PRIOR TO REACHING THE CENTER TAXIWAY (HEADING WEST OVER THE RUNWAY) AT 200 FEET AGL AND 105 KNOTS, THE PILOT MENTIONED TO THE COPILOT/GUNNER THAT THE PROBLEM WAS "PYLON ROCK" RATHER THAN A LATERAL VIBRATION. IMMEDIATELY FOLLOWING THIS STATEMENT, BOTH PILOTS HEARD A LOUD NOISE FROM THE TAIL BOOM AREA OF THE AIRCRAFT. THE AIRCRAFT DECELERATED SLIGHTLY AND STARTED A SLOW CLIMBING RIGHT TURN. THE PILOT APPLIED FORWARD CYCLIC IN AN ATTEMPT TO STREAMLINE THE AIRCRAFT; HOWEVER, BY THIS TIME THE AIRCRAFT HAD LEVELED AND WAS STARTING TO SPIN TO THE RIGHT. THE PILOT REALIZED THAT DIRECTIONAL CONTROL WAS LOST, SO HE ROLLED OFF THE THROTTLE AND ENTERED AUTOROTATION, AT WHICH TIME THE AIRCRAFT ATTEMPTED TO STREAMLINE. THE PILOT SAW THE TREES COMING UP AND HE PULLED FULL PITCH TO CUSHION THE AIRCRAFT INTO THE TREES. AS PITCH WAS PULLED, THE AIRCRAFT STARTED TO SPIN TO THE RIGHT AGAIN. THE AIRCRAFT CLEARED THE TREES AND LANDED IN A SLIGHTLY NOSE LOW ATTITUDE ON A MODERATE SLOPE TO THE AIRCRAFTS LEFT. THE RIGHT SKID BROKE ON IMPACT AND THE BOTTOM RIGHT FRONT OF THE AIRCRAFT CONTACTED THE CREST OF THE SLOPE THE MAIN ROTOR CONTACTED THE GROUND AT LOW RPM AS THE AIRCRAFT ROLLED ON ITS LEFT SIDE. THREE OF THE TAIL BOOM SUPPORT STUDS BROKE ON IMPACT, AND THE AFT END OF THE TAIL BOOM CAME TO REST IN THE CANAL. THE AIRCRAFT CAME TO REST ON A HEADING OF 050, 400 METERS WNW OF THE RUNWAY, AND IN A REVERSE DIRECTION FROM THE ORIGINAL FLIGHT PATH. THE PILOT EXITED THE AIRCRAFT BY THE RIGHT DOOR; HOWEVER, THE COPILOT/GUNNER HAD TO BREAK THE CANOPY TO EXIT THE AIRCRAFT. AFTER THEY WERE BOTH SAFELY OUT OF THE AIRCRAFT AND SAW NO FIRE WAS APPARENT, THE COPILOT/GUNNER RETURNED TO THE AIRCRAFT AND SHUT OFF THE FUEL AND BATTERY SWITCHES.
