Accident Summary:
THE AIRCRAFT INVOLVED IN THE ACCIDENT WAS A UH-1B GUN SHIP SERIAL NUMBER ^62-12516^. THE CREW CONSISTED OF THE AIRCRAFT COMMANDER, ^WO-1 LYNN L. BEAVERS, W3160690^; THE PILOT, ^1LT RONALD R. HOLLAND O5427582^; THE CREW CHIEF, ^SP/5 WILLIAM L. ROPER^ ^RA15871248^; AND THE GUNNER, ^SP/4 GARY H. ZAMMITO, RA18977392^. THE ACCIDENT OCCURED AT DUC HOA AAF, REPUBLIC OF VIETNAM, AT APPROXIMATELY 1400 HOURS ON 25 APRIL 1969. THE AIRCRAFT SUSTAINED SUSTANTIAL DAMAGE TO THE MAIN ROTOR BLADES, TRANSMISSION,ENGINE AND LEFT SIDE OF THE MAIN BODY. NO INJURIESOCCURED IN THIS ACCIDENT. UH-1B ^#62-12516^ WAS FLYING A VISUAL RECON MISSION AND HAD MADE AN APPROACH TO THE DUC HOA AIRFIELD FOR REFUELING AFTER A ONE HOUR FLIGHT. THE AIRCRAFT HAD APPROXIMATELY 600POUNDS OF FUEL AND NEARLY FULL ARMAMENT. CLEARANCETO LAND ON RUNWAY 06 HAD BEEN GIVEN BY THE TOWER OPERATOR, ^SP/5 RUESCLER^. WINDS WERE ESTIMATED TO BE SOUTHEAST AT APPROXIMATELY 10 TO 15 KNOTS. AFTER LANDING TO A HOVER, ^WO-1 BEAVERS^ MANEUVERED THE AIRCRAFT INTO THE POL AREA. HE CHOSE TO USE POINT NUMBER TWO AND HOVERED TO THIS POINT. THE AIRCRAFT WAS SET DOWN HEADING IN A GENERALLY NORTHERLY DIRECTION WITH THE RIGHT SKID ON THE SSP PAD AND THE LEFT SKID ON THE SURROUNDING GROUND. ^WO-1 BEAVERS^ CHOSE TO PLACE THE AIRCRAFT IN THIS POSITION AS HE FELT THAT INSUFFICIENT ROOM EXISTED TO PLACE THE AIRCRAFT PARALLEL TO THE RUNWAY. ANOTHER AIRCRAFT, A UH-1C, WAS REFUELING ON POL POINT NUMBER THREE AND WAS PARALLEL TO RUNWAY 06. DUE TO THE POSITION OF ^WO-1 BEAVERS'^ AIRCRAFT THE FUEL HOSE FROM POL POINT TWO DID NOT REACH FAR ENOUGH TO REFUELHIS AIRCRAFT. ^WO-1 BEAVERS^ THEN RETURNED HIS AIRCRAFT TO A HOVER AND BEGAN TO MOVE HIS AIRCRAFT TOTHE LEFT. THE CREW MEMBERS OF ^62-12516^ ALL AGREETHAT A ONE HALF TO ONE FOOT HOVER EXISTED BEFORE THE AIRCRAFT WAS MOVED TO THE LEFT. NO RADIO TRANSMISSION WAS MADE BY ^WO-1 BEAVERS^ TO THE EFFECT THAT HE WAS REPOSITIONING IN THE POL AREA. WHILE MOVING THE AIRCRAFT TO THE LEFT ^WO-1 BEAVERS^ STATESTHAT THE AIRCRAFT ON POL POINT THREE INCREASED ITSRPM TO 6600 WHICH CAUSED TURBULANCE TO AFFECT HIS HOVERING AIRCRAFT ^62-12516^. THE CREW MEMBERS OF ^62-12516^ HAVE STATED THAT THE COMBINATION OF ROTOR TURBULANCE CAUSED THEIR AIRCRAFT TO DESCEND FROM ITS APPROXIMATE ONE FOOT HOVER AND TO DIP TOWARDS THE HEEL OF THE LEFT SKID. AS THIS LOSS OF HOVERALTITUDE AND DIPPING TO THE LEFT REAR OCCURED, THEHEEL OF THE LEFT SKID MADE CONTACT WITH THE SSP MATTING TO THE LEFT OF POL POINT TWO. THE AIRCRAFT TILTED TO THE LEFT. ^WO-1 BEAVERS^ ATTEMPTED TO STOP THIS TILTING ACTION BY REDUCING COLLECTIVE AND ADDING FULL RIGHT CYCLIC. THE CREW MEMBERS STATE THAT THE AIRCRAFT STABILIZED MOMEMTARILY AND THEN TILTED TO THE LEFT EVEN FURTHER CAUSING THE MAIN ROTOR BLADES TO STRIKE THE SSP MATTING. AS THE AIRCRAFT ROLLED TO THE LEFT THE CREW CHIEF ^SP/5 ROPER^,BEGAN TO FALL FROM THE LEFT SIDE. HE ATTEMPTED TO REENTER THE AIRCRAFT BUT COULD NOT DO SO. HE THEN RELEASED HIS SEAT BELT AND TRIED TO RUN FROM THE AIRCRAFT. DURING HIS ESCAPE ATTEMPT ONE OF THE MAINROTOR BLADES STRUCK HIS HELMET WITH A GLANCING BLOW. ^SP/5 ROPER^ DID NOT SUSTAIN ANY INJURIES. THE OTHER CREW MEMBERS ESCAPED FROM THE AIRCRAFT AFTERTHE ENGINE WAS SHUT DOWN. THE AIRCRAFT DID NOT CATCH FIRE.
